Referral Program

Once the patient has completed an exam with Lil Smile Builders, simply complete the referral form. You will then receive a Starbucks gift card and be entered into a drawing for a $100 gift card, one grand prize winner is drawn each month!

There are 2 other easy ways to qualify:

1. You can stop by our office anytime during business hours to pick up referral cards from the front office. Dont forget to put your first, last name and phone number on the referral cards, before handing them to your friends and family.
